CHAPTER RECOGNITION INSTRUCTIONS

The "Application / Reports" tab provides a series of online forms that members can use to apply for chapter recognition annually. The Chapter Recognition Chair can use the online forms for the chapter selection process if they choose.

Note: Chapter Meritorious Service, Retiree Service and 25 Year Tenure recognitions are not submitted online.

When a member completes an online recognition form the information is stored in a database on the national website.The form requires the email of the Chapter Recognition Chair in order insure that the recognition chair receives a copy of the completed application form

When chapter recognition forms are submitted online copies are automatically sent to:

    1. The individual submitting the form.
    2. The Chapter Recognition Chair
    3. The National ESP Office.

Chapters choosing to use an alternate submission system must complete an online form for each nominee submitted for national / regional consideration.

Nominees who have not submitted an online nomination form will not be
considered for national recognition.

A list of chapter recognition recipients nominated for national / regional recognition is submitted by the Chapter Recognition Chair using the National Recognition Nomination Submission Form, located under the "National Recognition" tab.  The National Recognition Nomination Submission Form is necessary for the National Office to identify which submissions from a chapter are  to be considered for regional or national recognition.

When the Chapter Recognition Chair submits the National Recognition Nomination Submission Form the National Office receives the form and verifies there is an online form for the nominee and that dues are paid by each nominee. The office then forwards the completed recogniton forms to chapters in the appropriate region for balloting and selection of national /regional recognition recipients.

Distinguished Service Ruby Recognition / Friend of Extension

Nominations for Distinguished Service Ruby Recognition are submitted using the form under the National / Regional Recognition tab on the same page with the other recognition applications.  Ruby and Friend of Extension Nomination forms are submitted to National Office.  Ruby applications go to the chapters for balloting.

The summary information included on the nomination form of nominees selected for national recognition will be used for the recognition presentation and for the recognition booklet .

Friend of Extension nominations are submitted using the form under the Chapter Recognition tab. A Statement of Support from the State Extension Director is required, using the form under the National Recognition tab. Copies of the Friends of Extension applications go to the Scholarships, Grants and Recognition Committee for balloting.
